    Job Description
    This role is based in Bogota, Colombia. Only applications in English will be considered.

    Apollo XCM is seeking a top-notch 'Patient Care Coordinator' who will play a crucial role in improving the patient journey by offering personalized support and guidance.

    Responsibilities:
    • Be the main point of contact for patients, caregivers, and healthcare providers, providing empathetic assistance and resolving inquiries promptly.
    • Coordinate patient appointments, referrals, and follow-up care to ensure timely access to medical services and resources.
    • Collaborate with interdisciplinary teams to optimize patient outcomes and care coordination.
    • Educate patients and caregivers on treatment plans, medications, and self-care strategies to promote health literacy and empowerment.
    • Maintain accurate patient records, documenting interactions, care plans, and progress notes in compliance with standards.
    • Identify and overcome barriers to care, advocating for patients' needs and facilitating access to services and resources.


    Requirements

    • Registered Nurse (RN) license in US or Colombia.
    • Previous experience in healthcare coordination, patient advocacy, or customer service.
    • Bilingual (Spanish and English) with strong proficiency in both languages. C1 level of English is required.
    • Ability to multitask, prioritize, and work effectively in a fast-paced environment.
    • Knowledge of medical terminology and healthcare systems.
    • Empathetic and compassionate with a patient-centered approach.
    • Customer service focus.
    • Enjoys interacting with people.
    • Detail-oriented and proactive.
    • Familiarity with startup culture.
